Consumer Care Partnerships (CCP) is a peer support
program that assists mental health consumers with
creating a community based support team.
The purpose of a CCP team is to support an
individual in their recovery by assisting them with
reaching some identified needs and goals. The team
process is strengths-based, which means the focus of
the team is on valuing and building on the abilities
and inherent worth of each individual.

What Is “The Team”?
The individual decides whom they would to be part of
their
team. Teams may consist of people from many
settings e.g., friends, family members, co-workers,
neighbors, church members, and anyone else the
individual requests to be on their team. People on
CCP teams are not paid to be there—they are involved
because they care and have made a commitment to work
together on behalf of the individual.
Who Determines Team
Membership?
Each individual decides who is on his/her
team. CCP facilitators are available to help
identify team members and to assist with bringing
team members together.
Where Are Team Meetings Held?
Teams meet in convenient, accessible, and neutral
locations such as a coffee shops, libraries,
schools, churches, and public meeting rooms. Every
attempt is made to meet wherever the individual
consumer feels most comfortable.
Meeting times and places are based on the
individual’s preferences. Typically, during the
first month or so, teams meet weekly. Later, teams
often chose to meet once or twice a month,
eventually teams meet as needed. Meeting times are
tailored to the individual and their needs. If
“things” change, so can the frequency of the
meetings.
Who Are The Facilitators?
CCP team facilitators are volunteer consumers and
/or community members in recovery that have been
trained to facilitate team meetings and to use their
personal life experience and skills to inspire hope
and promote wellness and recovery.
What Is The Facilitator’s Role?
CCP facilitators provide support and encouragement.
They are not trained to be case managers, crisis
workers, therapists, or any type of mental health
professionals. Their role is to assist an individual
with creating a team and developing and implementing
a plan that meets the individual’s needs, and to
assure the designed plan is based on the unique
strengths, values, and preferences of the
individual.
What Is An Action Plan?
An Action Plan is the plan created by the individual
and the team to meet some identified needs and
goals. The plan is creative and flexible,
individualized, and changes with the consumer’s
needs. The consumer and team members put into
operation the steps of the plan.

What Is Expected Of Team Members?
All team members are asked to be present at as many
team meetings as possible and to bring their
resources, skills, and knowledge to the table. The
purpose of each meeting is to establish and nurture
relationships in which everyone is able to
contribute to the goals of the individual.
Individual strengths and talents are multiplied when
members join forces.
